Any laymen can be shown a cavity "sticking" with a dental instrument called an explorer. Or if a tooth needs a root canal, this is indicative of an abcess or infection, which is also evident if pointed out on an x-ray. And YES it is necessary to put fillings in primary (baby) teeth if they are decayed. Decayed primary teeth if not treated can affect the development of the permanent teeth. Not only to mention a primary tooth can also abcess just like a permanent tooth. Why take a chance for a child to experience the severe pain that an abcess brings? Also, if a child needs treatment, and the parent refuses to comply, in some states this can be reported as neglect.

probably alot especially on those kids that have medicaid because they cant sucker those who have to pay out of their pocket.

Any check by Dentist in his procedure whether necessary or not depends on the doctors oath to his profession and expertise.
